I have also been researching this option. I have -12.5 and -13D, and the doctor at Stanford (who is involved in the FDA study) said I was a "perfect candidate" for PIOL. I was told by another doctor, who had no actual experience with PIOLs, that some laser surgery might be needed after implantation. Because they are not customized to one's exact eye, PIOLs will not fix astigmatism or other such things. As far as I can tell there are few risks other than the (large) fact that it is real surgery going *into* your eye. However, the PIOL can be removed if there are complications.
