Could the reason for a majority of the necessary enhancement procedures be that, prior to the initial surgery, the contact lenses (soft and rigid-style) were not removed sufficiently long enough?

Yes, if you haven't removed your contacts long enough either before the initial exams or for the time between the exams and surgery, then just like a watch on a wrist leaves an imprint the same way your measurements or your surgical results will not have the correct calculations due to your eyes not accurately revealing the correct amount of tissue that has to be removed.
