A central island describes a cornea that after laser vsion corrrection has a small, central area of undercorrection. The goal of laser vision correction for myopia is to make the cornea flatter. In a small percentage of cases, most of the cornea is flattenned except for the very center (thus the name central island). Central islands can cause some difficulty with vision but they sometimes will resolve on their own. If the central island persists, they can be treated with an enhancement.
I hope this is helpful
