I had lasik surgery two years ago in British Columbia with follow-up in
Bellevue, Washington. The company name was Lasik Vision Canada. I've found
that the Bellevue location was abruptly shut down. I'd like to find my
screening records to provide to a new surgeon because my vision has changed.
Do you have any idea how I can track down those medical records? A newspaper
article indicates that the Bellevue location had some sort of relationship
with a company called Icon Laser, but the link from your website to this
company doesn't work.
I now live in Boise, Idaho and can't easily take a trip to Vancouver's
suburbs searching for the office (i've lost the address.)
Thank you for any guidance you may have.

My apologies. I would not know how to contact them or who now has their files. You can try looking up other eye doctors from the same area via the internet and see if they know - as I am sure that many of the patients from the bankrupt company had to find new doctors.
